2008 Spinal Cord Injury Hall of Fame Nomination Form
Thank you for participating in the 2008 SCI Hall of Fame nominations process. We invite you to nominate candidates in any or all of the 17 categories by completing this form. Individuals may be nominated in more than one relevant category, but each nomination must be submitted separately. Please do not resubmit the same nomination more than once as only one submission per category from each IP (internet protocol) address will be considered.
We suggest you review the entire form before starting, as you may wish to do some information gathering and writing offline first. Nominations must be complete, including contact information for both the nominee and nominator in order for this nomination to be considered (required fields are in bold). In drafting your submission, keep in mind that answers to all questions are very important and in many cases will be the only reference NSCIA members will have as a basis for their voting selection. In general, answers should be clear, concise and readable (you may find it helpful to write the description “offline” so you can review, proof and spell-check it, then paste it into this form). You may include references and/or hyperlinks to additional information (i.e. news articles and websites), but we recommend you do not rely on these as many readers will not click through and will not have the benefit of the additional information as they cast their vote.
